Procedures to Register / Enroll

2.  Documentation required of the parent or legal guardian at the time of registration:

  • Two verifications of legal residency from the following (must contain your street address):
  1. filed homestead exemption application form
  2. mortgage documents or property deeds
  3. apartment or home lease
  4. utility bills
  5. driver license
  6. voter precinct identification
  7. automobile registration
  8. affidavit of residency (if you are presently living with someone)
  9. district representative personal visit
  • Student's Birth Certificate
  • Certification of Immunization Compliance Form 121 (This form may be obtained from the health department or your pediatrician.)  *Out of state transfers must take the out of state immunization record to the Lee County Health Department at 532 South Church Street in Tupelo to be issued a Mississippi Immunization Form.
