Procedures to Withdraw

• The parent or legal guardian must contact the school counselor in person on the day of withdrawal.
• The student must return all textbooks and library books to the media center.
• The office personnel will complete the withdrawal form including:

1. Grades
2. Number of days absent
3. Name of the school to which the student is transferring
4. Media center release
5. Textbook release

• The parent or legal guardian will sign the withdrawal form.  A copy will be provided for the school in which the student will be attending. 
• If the student is moving out of state a copy of the immunization records will be provided.
• Official records will be sent to the new school upon request.
