Procedures to Withdraw
- The parent or legal guardian must contact the school counselor in person on the day of withdrawal.
- The student must return all textbooks and library books to the media center.
- The office personnel will complete the withdrawal form including:
- Grades
- Number of days absent
- Name of the school to which the student is transferring
- Media center release
- Textbook release
- The parent or legal guardian will sign the withdrawal form. A copy will be provided for the school in which the student will be attending.
- If the student is moving out of state a copy of the immunization records will be provided.
- Official records will be sent to the new school upon request.
